Planning Your Appointment
Planning ahead before your visit to Dr. Fenkell’s office will help make your experience as effective as possible. It is important that you give us the information that we need so that we can gain the best understanding of your problem and tailor your treatment to your needs.
Before your arrive?
You will receive an email prior to your appointment to register for your online portal. The online portal will contain forms which will help facilitate your evaluation- no printing is necessary. You can complete these forms at home on a device of your choice or in the office when you arrive for your appointment. If planning to fill out the forms at the office, please allow for an additional 10-15 minutes to complete the forms prior to your appointment. These forms only need to be filled out once on your first visit unless there are any significant changes or updates at future appointment. If you have not received this email please check your junk/spam inbox.
If you do not wish to sign up for the portal system, click the New Patient Form, print and fill out form, and bring to your visit.
What should I bring with me?
As a general rule of thumb, the more information you bring to your appointment the better it will help us give you a diagnosis, prognosis and the best recommendations available to you.
First Visit
When you come for your appointments, please bring the following:
Driver’s License or a valid ID
Insurance information
Referral Letter (if required)
Reports, X-rays, MRIs, CT scans, etc. and any other relevant information. If possible please bring a physical copy of any of these tests/scans.
List of medications (if any)
Your medical records.
Your surgical and operative reports.
A list of allergies and any reactions you have to medications.
Any questions or concerns you have about your current condition.
